The 8am accident at the Jalan Song Ban Kheng-Jalan Chian Heng Kai intersection was caught on the dash cam of another car.
The footage shows how the Nissan Urvan ambulance, with lights flashing and alarm blaring, was first T-boned by a Perodua Kancil and a Honda City as it crossed the road. The two cars were crossing when the lights were green.
The sheer impact of the two cars forced the ambulance onto another lane where a group of about 10 motorcyclists were queuing up at the lights.
The ambulance was seen crashing into three motorcyclists who were shoved off their machines upon impact.
Police say the 40-year-old ambulance driver was unhurt and was responding to an emergency call from Simpang Ampat. There was no patient inside.
Central Seberang Perai police chief ACP Nik Ros Azhan Nik Abdul Hamid said a 43-year-old motorcyclist and two others received outpatient treatment at the Bukit Mertajam Hospital.
He said the case will be investigated under the Road Transport Act 1987.
